What do I need to send to you to approve my charge?

For HRAs - we require the Explanation of Benefits from your insurance provider For FSAs - we require the Explanation of Benefits from your insurance provider or a detailed statement or invoice from your provider that includes the following: 1. Date of Service 2. Patient Name 3. Provider Name 4. Amount due (after insurance, if applicable) 5. Services performed For DCAP - we require a receipt with the dates of sevrice and the caregiver's EIN/SS# For Parking & Transit - we require the parking or transit receipt with valid dates. If parking in a facility that does not generate receipts, you must declare the dates you parked in facility.
