If your employer offers a benefits card and the dependent is over 18 years of age, an additional card may be ordered for your dependent.  

 Login to order an additional card or contact BBP Admin and have the following information ready:
1. Dependent Name
2. Relationship
3. Date of Birth
4. Social Security Number
5. Dependent Address (if different)
